Trying to catch up. Did SpaceX personnel activate both FTS, just one, or neither?
Both stages were destroyed by the FTS. Commanded or autonomous we don't know yet. I would bet both were autonomous. Looks like the rotation of the booster so it could do it's "boost back" burn was faster than expected. This led to the fuel sloshing in the tanks, so when booster tried to relight the center 10 engines, that didn't work. So FTS took it out.

Starship, or just Ship. Less info. It separated clean and proceeded downrange IAW the flight plan, so most likely engines were good at least initially. Nothing more yet.

Ah, the latest production diagram, which answers "What's next after destroying the second set of test articles?"

The diagram indicates degree of completion, with metallic colors indicating this item is indeed at that stage of completion.

Two things of note here. One is it when your material is cheap as hell meaning stainless steel, your test articles don't cost so much. Two is the reference to the end of the V1 series, which makes you wonder what changes are going to be made for V2.

Oh and possibly not the best choice for nomenclatures for rockets... :)
